Here are the step-by-step instructions to apply for an Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PM-JAY) card if you live in Punjab:
Step 1: Check your eligibility
In Punjab, the following categories are eligible for the PM-JAY scheme:
- Rural families identified as per the Socio-Economic Caste Census (SECC) 2011 data
- Urban families with a yellow/pink/blue ration card
Step 2: Gather required documents
- Ration card (if available)
- Aadhaar card of all family members
- Bank account details (optional)
- Mobile number
Step 3: Visit the nearest PM-JAY Kendra/CSC center
Locate the nearest PM-JAY Kendra or Common Service Center (CSC) in your area. You can find the list at https://beneficiary.nha.gov.in/
Step 4: Submit application form
At the center, submit a physical application form along with the required documents. The center will verify your details.
Step 5: Provide biometric authentication
Your fingerprints/iris scan will be captured for biometric authentication.
Step 6: Get e-card/print letter
After verification, you will either get a PM-JAY e-card generated on the spot or a printout confirming your eligibility.
Step 7: Check status online
You can also check your application status online at https://beneficiary.nha.gov.in/ using your mobile number.
Alternatively, you can also apply online directly at https://mera.pmjay.gov.in or through the Mobile App by providing requisite details and uploading documents.
The whole process is free of cost. Once approved, your PM-JAY card will allow you to get cashless treatment worth Rs. 5 lakh per year at any empaneled hospital across India.
