Here are the key details about the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PM-JAY) scheme in Gujarat:

Eligibility:

In Gujarat, the following categories are eligible for PM-JAY:

- Rural families identified based on Socio-Economic Caste Census (SECC) 2011 data

- Urban families with yellow/orange/pink/blue ration cards

Implementation:

The scheme is being implemented by the Gujarat State Health Agency under the National Health Mission, Gujarat.

Empaneled Hospitals: 

As of March 2023, a total of 1,018 hospitals (387 public and 631 private) have been empaneled under PM-JAY in Gujarat. Some major empaneled hospitals include:

Public:

- Civil Hospitals across districts

- U.N. Mehta Institute of Cardiology (Ahmedabad)

- Gujarat Cancer & Research Institute (Ahmedabad)

- B.J. Medical College (Ahmedabad)

Private:

- Zydus Hospitals (Ahmedabad, Anand, Rajkot)

- Apollo Hospitals (Ahmedabad, Gandhinagar)

- HCG Cancer Hospitals (Ahmedabad, Surat)

- Muljibhai Patel Urological Hospital (Nadiad)

PM-JAY Beneficiary Numbers:

As per the National Health Authority dashboard, around 65.4 lakh PM-JAY beneficiary families have been identified in Gujarat as of March 2023.

Gujarat has issued PM-JAY e-cards to eligible beneficiaries and set up a robust network of empaneled hospitals to provide cashless treatment up to Rs. 5 lakhs per year under the scheme.
